I notice one form is 2010 and the other is 2011. Do you know if this is going to be a requirment now, or something they can throw at you any given time.

Hey, these things are brand new so there can be no 2010 or 11 versions. Those are preview dates. The final stuff was released Friday.
They, the VA seems to be making it harder for the Doctors to help the Veterans. Not many Doctors would take the time. Looks like the Veteran would have to fill all in and then ask for a signature.
Perhaps I should put up a link to the ICD codes. I had to do that when I sent in my insurance disability form. NY Life wanted a list of all my diseases and their corresponding codes. When has the government ever made anything easy for us? This may become the default nexus letter. It certainly resembles one.
